   1 year – GHS 1410
   2 years – 6 years – GHS 1638
   7 years – 10 years – GHS 1788
   11 years – 14 years – GHS1788
   15 years – 17 years – GHS2718
   18 years – GHS 2952
   19 years – 24 years – GHS 2852
   25 years – 44 years – GHS 2808
   45 years – 64 years – GHS 2688
   63 years – 64years- GHS 2688
   65 years and above – GHS 2378
   Pregnant – GHS 2128

PLEASE BRING YOUR

2. Original passport (valid for at least 6 months)
3. Photocopy of the passport
4. Appointment letter for your visa interview or letter instructing you to have a medical exam done (printed out)
5. Your DS-260 or DS-160 confirmation page.
6. COVID 19 VACCINATION RECORDS. (15yrs and over)
7. Vaccination records if available
8. Weighing card for children 6 years and below

THE FOLLOWING ARE ALSO REQUIRED :

10. Residential address in Ghana.
11. Intended address in the United States of America.
12. Working time is Mondays-Fridays 8am-3PM
13. Saturdays 8am-2PM
14. It is a walk-in no appointment needed.
